Liability Waiver & Medical Authorization

By signing below, I acknowledge, agree to, and accept the following on behalf of my child(ren) registered with Amanda's Dance Center:

I acknowledge that dance, acrobatics, tumbling, rehearsals, performances, camps, and related activities involve inherent risks including serious injury. I voluntarily allow my child to participate and assume all known and unknown risks. To the fullest extent permitted by Florida law, I release Amanda's Dance Center, its owners, instructors, employees, volunteers, and agents from claims arising from ordinary negligence. This release does not apply to gross negligence or intentional misconduct.

I authorize staff to provide basic first aid, contact 911, and obtain emergency medical treatment if I cannot be reached. I accept responsibility for any and all related medical expenses.

Media Release

By signing below, I grant permission for Amanda's Dance Center to use photos and videos of my child for the website, social media, advertising, and promotional materials. Parents who prefer to opt out may contact the studio director directly.

Parent Portal Acknowledgment

I understand that access to the Amanda's Dance Center Parent Portal will be provided after registration is complete. By signing below, I agree to review the Parent Portal once access is granted and to follow all information and policies it contains, including tuition, attendance, dress code, recital, and studio policies.

Electronic Signature

By signing below, I agree that my electronic signature is legally binding and constitutes my acceptance of all terms above.
